Easy. Assuming the amp has a 50 ohm, unbalanced output:

Amplitude-modulate your audio, feed the amp's output through a balun
to make it balanced, followed by a full-wave bridge rectifier, then a
single-ended audio transformer (which also serves as an RF choke).

Impedance matching, however, is a big ball of very sticky wax.
