do you hook them directly to the handset or what?

There is an interface box with a SO239 and power leads coming out, also an
external speaker jack. Hide the box if you want, also comes with a mounting
flange for the conection whip so you can "plug into the dash"

Yep, comes with it, Not much in there, just power, ext speaker switching,
filter network and box. that part is sold as a spare from Cobra so the
handheld unit can be transferred between different vehicles.
