Think of the Red Brigades or the Weathermen. While there certainly are terrorists in the poorest parts of Central and South America, Africa and Asia, they tend to stay put and limit their horrible crimes to their neighbors. The suicide bomber seems fairly recent to me. I first became aware of the tactic with the bombing of the Marine barracks Beirut. Suicide bombings seem like a desperate, last resort tactic.
