How small could the PCB be?

I've seen them as small as 1 micron.

What is the minimum size for a pcb printed GSM
antenna?

Many orders of magnitude larger than 1 micron, certainly.

Your problem is defining the di-electric constant, and its
contribution to altering the size of a conventional wire antenna in
free space.
