insurance policy remain in force when I revoke the authority for the
insurance company to access, or use, my personal information, after I

first name, last name, to use my personal identification information,
for any insurance policy, or other reasons, regarding any past,
present, or future products that you sell or administer, after this
point in time, any use of my personal information after this
notification will violate my privacy rights granted under the Privacy
Act of 1974, and as such, you must permanently destroy, and no longer
use, my personal information for any reason.
Signed, the oppressed citizen who is calling to God to stop this
oppressive government from intruding into my life

Dear Employer
Effective today, xx/xx/xx, you are no longer authorized by myself,

authorize any health care plans, for any reason.
The reason for this revocation is the affliction without compensation
the burden of maintaining by government illegal mandate to purchase or
pay taxes on a product that I do not want, which violate my fifth
amendment, civil rights, and paying taxes on such a product, or
purchasing of such a product, will force me to be financially
negligent in my personal affairs, of which, is my responsibility and
not the government's.
