Heat sinks depend on surface area to dissipate heat. They put fins on them to increase the amount of surface area. Maybe put it in one side of the case and drill holes on the other and have a cross wind over the pills. You might try doing what computer manufacturers do and look for heat sinks to put directly on the ceramic IC package.
