Looks like some kind of internal thermal issue.
Notice the third pin ground, unusual in wall warts, and if you get a
good one it should be quieter than the simple two blade types.


Most of the wall warts I've worked on that had the ground pin on them,
didn't have that pin connected to anything inside. Basically, it was there
to help hold the wart in the outlet.
